Abstract:
Aims: Ob.1) To describe the processes of social comparison in patients with fibromyalgia, Ob.2) To analyze the processes of social comparison in terms of sociodemographic and clinical variables, Ob.3) To describe the catastrophizing, fibromyalgia illness impact and psychological distress (anxiety and depression) levels as health outcomes in our sample of fibromyalgia patients Ob.4) To analyze psychosocial variables (i.e. catastrophizing, fibromyalgia illness impact and psychological distress (anxiety and depression) in terms of sociodemographic and clinical variables, Ob.5) To identify distinct patient profiles according to painperception, the use of social comparison and the emotional responses and Ob.6) To evaluate through a path analysis in a sample of Fibromyalgia patients the association between identification and contrast in social comparison, catastrophizing, and specific health outcomes (fibromyalgia illness impact and psychological distress). Method: 131 Fibromyalgia outpatients from Fibromyalgia Service (UAI-FM) - Hospital de San Vicente del Raspeig (Alicante) were assessed (mean age: 50.15, SD = 11.1). Average years since onset of symptoms and 4.32 (SD = 4.99) average years since diagnosis until their arrival at the Fibromyalgia service. Regarding pain-perceived, mean scores were for current pain, M = 5.48, SD = 1.56; for average pain, M = 6.58, SD = 1.56; and for maximum pain, M = 7.18, SD = 1.44 (range from 0 to 10). Results: Ob1) 75.6% of our patients used upward contrast with high frequency (often/very often), which was the most used, with M = 11.30 (SD = 3.11; Range = 3- 15), followed by downward identification, used with high frequency in 56.5%, and M = 9.98 (SD = 3.63; Range = 3-15). Ob5) Two profiles according to their adaptability are presented. The first profile (66%) is characterized by higher pain-perceived (p < .001) and high levels of anxiety and depression (p < .001) together with more upward contrast and downward identification comparisons (p < .001). Second profile (34%) presents less pain-perceived (p < .001) and lower levels of anxiety and depression (p < .001) together with more use of upward identification (p < .001) and downward contrast (p < .05) comparisons. Ob6) We present a model that explained 33% of the variance in catastrophizing by direct effects of more use of upward contrast and downward identification. In addition, 35% of fibromyalgia illness impact variance was explained by less upward identification, more upward contrast and more catastrophizing and 42% of the variance in psychological distress by a direct effect of more use of upward contrast together with higher fibromyalgia illness impact. Conclusions: Features of our simple are representative. Our sample of women presents catastrophizing, fibromyalgia illness impact and psychological distress (anxiety and depression) representative and similar to those found in other Fibromyalgia patients. Women with Fibromyalgia use most social comparison strategies quite often. However, upward contrast and downward identification comparison are the most used strategies by these patients in a unfavourable comparison pattern. Our results support the presence of different profiles of Fibromyalgia women. We assessed a suitable model. Their findings suggest that unfavorable comparison strategies together with catastrophizing somehow determine Fibromyalgia emotional and impact results. The downward contrast strategy has no relation to the model.
