Background: To prevent SARS-COV-2 infection to progress to severe disease, prevention and early treatment therapies have been established. They reduce hospitalizations, sequelae and deaths associated with the disease. However, underuse of these drugs has been reported. Objective: To conduct a clinical audit of the use of early treatment of SARS-COV- 2 infection in patients hospitalized. Analyse possible reasons for the underuse of this treatment and their consequences. Methods: Prospective longitudinal study of hospitalized COVID-19 patients from April 2022 to February 2024. Treatment delay from the onset of symptoms, clinical characteristics, inpatient treatment received, as well outcomes are described. Results: Two-hundred and four patients were evaluated. A total of 87 patients were estimated to have early treatment criteria, but none received it. Inpatient death rate was 6.9%.Conclusion: A diagnostic and therapeutic delay in COVID-19 in high risk patients hat reduces the rate of early treatment, increasing comorbidities and the death rate.
