Inguinal hernia is a common pathology in Spain, with significant repercussions on the active population and on healthcare and social costs associated with prolonged recovery. Our study aimed to identify variables affecting return to work in patients under 65 years of age. The results revealed that a hernia size larger than 1.5 cm was associated with a longer hospital stay (2.73 days vs 1.63 days, p = 0.027) and an extended return-to-work time (37.71 days vs 19.50 days, p = 0.023). Hernia incarceration also prolonged hospital stay (4.10 days vs 2.09 days, p < 0.001). Regarding surgical techniques, the Liechtenstein technique was associated with a shorter hospital stay (2.20 days vs 3.92 days, p = 0.038), while the Plug and Patch technique showed prolonged stay (4.47 days vs 2.19 days, p = 0.023). No significant relationship was found between type of employment (working for a company or self-employed) and return to work. In conclusion, the duration of surgical intervention, body mass index (BMI), and age were identified as determining factors in return-to-work time in active patients under 65 years of age. Additionally, the Liechtenstein technique showed advantages in terms of faster recovery compared to the Plug and Patch technique. These findings underscore the importance of considering these factors when planning and addressing return-to-work rehabilitation for patients with inguinal hernia.
