Background: The aim of this study is to do a review of the conservative management of endometrial cancer.
Materials and methods: We reviewed the available literature about the conservative management of endometrial cancer and it was completed by reviewing the cases of women that had a conservative management in the General University Dr Balmis Hospital of Alicante in the last five years.
Results: From our bibliographic research we obtained multiple studies that verse about fertility-sparing management for women with endometrial cancer, and all of them agree that this kind of treatment it’s an option in specialized centers in cases where there will be a close followup of the patient to check on the level of response and the eventual complications, as well as the fact that fertility treatments are oncologically safe, with good pregnancy rates in the women with incomplete genetic desire that, after the pregnancy, can then go on to have the definitive surgical treatment of histerectomy and ooforosalpinguectomy. When it comes to the women that are not surgical candidates because of elevated BMI or other comorbidities, literature has shown that the women that achieve a weight reduction have better oncological results, with a better chance of response and less chances of recurrence. Of the treatments available for both groups, the one that stands out and seems to have better response rates and convenience is the one where a histeroscopic resection is performed and a IUD of levonorgestrel placed. Of the cases of the HGUA, results are similar to those of the studies, all of the patients are alive and have responded to treatment, and all of them have been treated with the IUD of levonorgestrel. Conclusion: Off the available literature and the cases of our hospital we can infer that the conservative management of endometrial cancer in selected patients and in specialized centers is oncologically safe, and an alternative when the patient wants to preserve their fertility or is not a surgical candidate because of elevated BMI or other comorbidities.
