Neck pain is a multifactorial condition with a prevalence of around 42 to 67% among young adults, although it is not the most common musculoskeletal disorder. In the following observational study, the aim was to investigate potential differences in the craniocervical angle between men and women, as well as to examine any relationship with nonspecific neck pain. A total of 44 volunteers participated, including 24 women and 20 men, of whom 27 had nonspecific neck pain at the time of analysis, with 19 of these being women. The participants underwent photogrammetry to perform anatomical measurements by taking photographs in standardized positions, which were later analyzed using the free software Kinovea. The results showed an average craniocervical angle of 43.52° for women and 48.96° for men; in both cases, the most painful area was the trapezius, with 45% in men and 75% in women. Additionally, there were statistically significant differences between sex and pain, with women experiencing more pain than men (p-value = 0.001 and p-value = 0.008). In conclusion, women have a smaller craniocervical angle and report more pain than men, which corresponds to a more forward head position compared to men.
