Background: Heart failure (HF) is one of the global healthcare challenges in our society. Individuals suffering from it are exposed to a higher risk of frailty and dependence, resulting in reduced physical exercise tolerance. All of this leads to a deterioration in the patient's quality of life, with an increased risk of mortality and hospitalizations, as well as a rise in healthcare costs.
Objectives: The main objective of this literature review is to determine the effects of cardiac rehabilitation (CR) on morbidity and mortality in HF patients by comparing the hospitalization and mortality rates between groups of patients who follow a CR program and those who do not. Other secondary objectives include assessing the impact on the quality of life of CR, through the evolution of the cardiopulmonary functional capacity of patients, as well as questionnaires such as the Minnesota Living with Heart Failure (MLHFQ), the Kansas City Cardiomyopathy Questionnaire, or the SF-36 health questionnaire; analyzing the population that can obtain the greatest benefit, studying potential differences in results based on patient characteristics; evaluating the applicability of CR programs for HF patients in Spain, considering the level of interaction between HF and CR units, and assessing the cost-effectiveness of the intervention.
Method: To achieve these objectives, a review of studies that analyzed the mentioned variables was conducted.
Results: The results of the selected studies regarding mortality and hospitalizations concluded that they were not significantly reduced in the group of patients who underwent CR. As for cardiopulmonary functional capacity and quality of life, there was a significant improvement in this group of patients. Differences in results were found with respect to gender, with lower referral and adherence in women, but the benefit of CR being equal or superior in this group. The cost analysis showed that CR was a cost-effective intervention.
Conclusion: Overall, it was concluded that CR is a recommended intervention for patients with HF, but its scope is still limited, and further studies demonstrating its benefits with higher quality are needed.
