Introduction: Fibromyalgia is a chronic idiopathic disease that generates widespread musculoskeletal pain, in addition to other symptoms such as fatigue or depression. Aerobic exercise is a treatment that has shown a great positive impact on this pathology, in addition to generating few adverse effects. Objectives: To carry out a systematic review with meta-analysis in order to analyze the effectiveness of aerobic exercise on pain, severity of symptoms and depression in fibromyalgia. Material and methods: A bibliographic search were carried out in the Pubmed and Embase databases, obtaining a total of 119 articles. After applying the corresponding filters, removing duplicates and applying the inclusion and exclusion criteria, 17 articles were selected for the systematic review with meta-analysis. Risk of bias was assessed using the Cochrane Risk of Bias tool and methodological quality was assessed using the Physiotherapy Evidence Database (PEDro) score. Results: The meta-analysis found evidence that aerobic exercise generates a significant reduction in the three intervention periods proposed for pain (SMD=0.83, SMD=0.84, SMD=0.53 respectively), the severity of symptoms (SMD=0.76, SMD= 0.80, SMD=0.55 respectively) and depression (SMD=0.89, SMD=0.80, SMD=0.88 respectively) compared to the control group. Discussion: Aerobic exercise shows positive effects on the variables analyzed. But it has not been possible to demonstrate which type of protocol is the most effective for the treatment of symptoms due to the lack of available evidence.
