Introduction: The prevalence of anemia in patients with heart failure is around 50% in hospitalized patients. Its diagnosis follows the same criteria as in the general population with hemoglobin below 13 in men and 12 in women, ferritin below 41 mg/ml and transferrin below 20%. Regarding the clinical symptoms in this type of patients, they usually present dyspnea and fatigue. Alternatives to treatment are oral iron supplements, intravenous iron, new drugs such as daplagliflozin, or red blood cell transfusion. Methodology: A bibliographic search of the articles that study the efficacy and safety of iron treatments for anemia in patients with heart failure has been carried out in different databases such as Pubmed. Also, the search has been carried out in evidence summaries, specifically in uptodate. Randomized clinical trials have been included, up to May 9, 2023. Results: 18 clinical trials have been selected. Regarding oral iron supplements, an improvement in the 6-minute walk test has been verified in patients treated with ferrous sulfate, with iron supplements being better tolerated. Treatments with intravenous iron have produced a decrease in the number of hospitalizations, an improvement in the quality of life, and a reduction in the hypercapnic ventilatory response. On the other hand, an improvement in hemoglobin levels has been verified in patients treated with intravenous iron, than with oral iron. Conclusions: Iron supplements are better tolerated than iron supplements. These supplements have a good tolerance and safety profile, increasing the exercise capacity and quality of life of patients with heart failure regardless of the presence of anemia.
