Introduction: Recent research shows that there is an immunoinflammatory basis in the
pathophysiology of psychosis. We wonder if the study of certain inflammatory
biomarkers could pave the way towards early diagnosis and treatment of first psychotic
episodes, thus improving the prognosis of acute psychoses.
Hypothesis: The study of inflammatory biomarkers in FEP patients may be useful for
early diagnosis and treatment of acute psychosis.
Objectives: We establish the objective of describing the main biomarkers studied and
justifying their applicability in clinical practice, as well as determining the role of
inflammation in the course and treatment of the disease.
Material and methods: A systematic review has been carried out to collect evidence of
the use of inflammatory biomarkers for the diagnosis and early treatment in PEP, using
as keywords "First episode psychosis", "Inflammatory", "Cytokines", "MRI",
“Schizophrenia”, “Clinical High Risk Psychosis” in PubMed, selecting 22 articles.
Results: The scientific evidence found so far suggests an immuno-inflammatory state in
FEP patients that is related to the pathogenesis, prognosis and treatment of the disease.
Conclusions: More studies of specific inflammatory profiles in stratified FEP patients are
required, as well as randomized clinical trials that compare the anti-inflammatory effect
of antipsychotics and possible new therapeutic tools.
