Introduction: Headache affects people of all ages, races, income levels and geographic areas and has a global prevalence of between 46 and 78.6%. Among the 3 most common types of headache are migraine, tension-type headache and cervicogenic headache. Conventional treatment is usually pharmacological, however, in recent years exercise has proven to be a tool with great potential to control this disease.
Objective: To know the effect of therapeutic exercise as a treatment for headache, as well as the optimal exercise modality for migraine, tension-type and cervicogenic headache.
Material and methods: A bibliographic search was carried out in the Pubmed, PEDro, Web of Science, Science Direct and Embase databases of clinical trials dealing with the efficacy of exercise as a treatment for headache.
Results: Fourteen studies were included, eight used resistance exercise and six used aerobic exercise as the main intervention. The most common outcome measures were headache frequency, duration, pain intensity, and frequency of analgesic drug use.
Conclusions: There is moderate-high evidence that both strength and aerobic exercise are effective tools for headache management. Better results were obtained with multimodal therapy than with exercise alone. More and higher quality studies are needed to identify the optimal volume, intensity, and modality of exercise for each type of headache.
