Abordaje precoz de descompensaciones por Insuficiencia Cardíaca en Atención Primaria. Programa PIICAP

Abstract

La Insuficiencia Cardíaca (IC) comprende una patología con alta prevalencia en España (1-2%) y genera una gran carga asistencial (3% de los ingresos). Comprende una causa importante de mortalidad (3.9% de las muertes en 2020). Atención Primaria (AP) supone el primer acceso al sistema sanitario y goza de cercanía hacia el paciente. Se ha demostrado la efectividad de estrategias de seguimiento de pacientes con IC en unidades específicas a nivel hospitalario. Objetivo: valorar la efectividad del programa PIICAP (Paciente Instruido en Insuficiencia Cardíaca desde Atención Primaria) en reducción de ingresos hospitalarios y asistencias al Servicio de Urgencias. Intervención: establecer un programa de formación de pacientes en IC y conseguir una optimización terapéutica conjunta entre facultativo y paciente con un seguimiento de ±24 meses. Se plantea alcanzar un control consensuado de la enfermedad dentro de una estrategia de paciente experto. Análisis estadístico: estudio cuasiexperimental antes-después con muestreo no probabilístico consecutivo. Se medirá la variable resultado como presencia de ingreso o agudización así como otras variables con carácter demográfico y explicativo. Se calculará el Riesgo Relativo derivado de la intervención así como el Número Necesario a Tratar. Se plantea un estudio de supervivencia y un estudio multivariante. Aplicabilidad: el estudio radica en la idoneidad de estrategias de promoción de la salud en el manejo de patologías crónicas desde AP y el empoderamiento de los pacientes. La iniciativa forma parte de los objetivos planteados en el Marco Estratégico de Atención Primaria y Comunitaria 2022/2023.

Heart Failure (HF) is a high prevalence disease in Spain (1.2%) and causing the care burden to increase (HF represents 3% of hospitalizations). Primary Health Care is the first step of the Health System, and it is close to the patient. Followup of patients in HF Units has demonstrated effectiveness at Hospital Care. Objective: to assess effectiveness of PIICAP programme (Heart Failure Educated Patient in Primary Care) and evaluate the reduction of hospitalization and emergency consultations. Intervention: a training programme for patients and family members on HF will be established. We want to achieve a consensual treatment optimization (patient and physician) with a twenty-four moths-follow up before hospital discharge. It will consist in an expert’ patient strategy. Statistic analysis: the project is a quasi-experimental before-after study with a consecutive non-probabilistic sample. Result variable will be measured as hospitalization or exacerbation. Intermediate variables will be measured as a demographic indicator. Intervention’s Relative Risk and Number Needed to Treat will be measured too. Survival data and multivariant study will be showed with statistically significant results of bivariant model. Relevance: the health promotion strategy adds value to this study and makes it a relevant one. The proposed plan allows an effective management of chronic diseases on Primary Care. Although, it fortifies share decision making. This trial empowers patients, and it integrates all of health care professionals (nursery, hospital caregivers, healthcare administrators…). The initiative takes part of the objectives showed at Primary and Community Care Strategic Framework 2022/2023.
