Objective: To evaluate the validity and diagnostic safety of the combination of clinical,
analytical and radiological criteria in the diagnosis of COVID-19.
Material and methods: Retrospective study of patients admitted for suspected COVID19 between March and April 2020. The sensitivity, specificity, positive predictive value
(PPV), negative predictive value (NPV) and positive likelihood ratio (LHR) were
calculated for the clinical (fever, cough, dyspnea, arthromyalgia, asthenia, hypoxemia and
anosmia / ageusia), analytical and radiological criteria, and their combination.
Results: 313 consecutive patients were included, 115 (36.7%) with positive RT-PCR and
198 controls with negative RT-PCR (63.3%). The combination of clinical, analytical, and
radiological criteria showed a sensitivity of 73.0%, specificity of 89.9%, PPV of 80.8%,
NPV of 85.2%, and positive LHR of 7.2 for the diagnosis of SARS-CoV-2 infection in
admitted patients.
Conclusion: The combination of clinical, analytical and radiological criteria allows a
reliable approach to the microbiological diagnosis of SARS-CoV-2 infection. This can be
relevant in situations of scarce resources and in case of doubts about the need of repeating
the RT-PCR if the first one is negative.
