Introduction: Cancer is one of the leading causes of death in the world and specifically, breast cancer is the one with the highest incidence among women. Therapeutic aerobic exercise is very beneficial after breast cancer treatment, being able to generate a better quality of life and decreasing the sequelae after cancer treatment. Methods: A search was carried out during the months of February and March 2021 in the Pubmed, Cochrane, Scopus, Embase and PEDro databases. Taking into account the search criteria, 25 trials were selected. Methodological quality was assessed using the PEDro scale. Results: According to the articles analyzed, aerobic exercise, alone or in combination, mitigates the sequelae of cancer, including fatigue, lymphedema, pain, musculoskeletal alterations, changes in body composition, cognitive deterioration and a decrease in functional capacity and quality of life. In addition, high-intensity aerobic exercise is safe for patients. Aerobic and resistance exercise is the combination shown to have the most benefits. More research is needed to observe whether aerobic exercise can prevent breast cancer in women at risk for breast cancer. Conclusions: Aerobic exercise has shown to have benefits in decreasing physical and psychological sequelae in BCS, thus improving patients' quality of life.
