Objective To determinate the long-term and short-term effectiveness of exercise therapy on quality of life, pain, physical function, strength and hip ROM in patients with mild or moderate osteoarthritis of the hip. Methods We conducted an extensive literature search in PubMed, Embase, Scopus and PEDro for randomized controlled clinical trials. The methodological quality of the studies was carried out through the PEDro scale, showing a high general quality in the studies. Results Thirteen high-quality and one regular-quality randomized controlled trials were included. We found evidence that there is no short-term and long-term effectiveness on pain, on ROM, or short-term on quality of life, and contradictory evidence for physical function. Long-term quality of life and strength could not be assessed due to a paucity of studies. Conclusion The effects produced by exercise therapy can be effective on physical function in patients with mild or moderate osteoarthritis of the hip in the short and long term. In quality of life, pain, strength and ROM the effect was limited or ineffective. More research is needed on exercise programs other than the more typical ones for strengthening, with an adequate dose and that are more adherent and functional, such as Nordic Walking.
