Objetive: To describe the drug therapy of Epilepsy in pregnant woman and how
pregnancy should be programmed taking intro account physiological changes
that happen and the drugs´s teratogenic effects.
Methods: A systematic review was carried out with articles published on the
Internet, in particular those published in the bibliographic databases MEDLINE
(vía Pubmed) and The Cochrane Library. To do the search in both databases
the following descriptors MeSH (Medical Subject Headings) have been used:
“Epilepsy”, “Drug therapy” and “Pregnancy” and the filters «Humans» and
«published in the last 10 years».
Results: Applying the equations mentioned above, 19 references have been
recovered in MEDLINE (Pubmed), which were published between 2008 and
2016 and one on The Cochrane Library, published in 2014. After applying
inclusion and exclusion criteria only six studies have been accepted (all
published in MEDLINE).
Conclusions: Monotherapy is recommended to the minor effective possible
dose and to avoid the use of phenytoin and phenobarbital during the first
quarter and the treatment with valproic acid and polytherapy and during the
whole pregnancy whenever possible.
During the gestation the women suffer a series of physiological changes that
affect drug pharmacokinetics and their serum concentrations. For that it is very
important to do frequent controls of the above mentioned.
Also it must be taken into account that the contraceptives that have estradiol
increase the metabolism of lamotrigine and valproic acid, so when the woman
interrupts the contraceptive treatment it is advisable to monitor the serum
concentrations of these drugs.
