Introduction: Malnutrition is an important health problem since it contributes to the appearance of numerous disorders that can cause a huge impact on the quality of life, as well as an increase morbidity and mortality and healthcare costs. Its prevalence in hospitalized patients is high, especially in elderly patients, which is why we consider the use of nutritional screening tools such as the MNA-SF in this population.
Objectives: The main objective is to screen nutritional status using the MNA-SF questionnaire in patients hospitalized in the Internal Medicine service. As secondary objectives, we plan to determine frailty, sarcopenia, probability of survival, grip strength and calf circumference and diagnose malnutrition using the GLIM criteria in patients hospitalized in this service.
Hypothesis: There is a high prevalence of elderly patients hospitalized in the Internal Medicine service with risk of malnutrition or undiagnosed malnutrition.
Material and methods: This is an observational, descriptive and cross-sectional study. Patients over 65 years of age admitted to the Internal Medicine service of the San Juan de Alicante hospital were evaluated during the period from October 2, 2023 to December 22, 2023. The MNA-SF score was collected as the main variable and as secondary variables the GLIM criteria for the diagnosis of malnutrition, clinical and anthropometric variables, dynamometry, FRAIL, SARC-F and PROFUND index.
Results: 31 patients were included, 15 men (48.8%) and 16 women (51.6%) with a median age of 81 years. The MNA-SF detected the presence of malnutrition in 13 patients (41.9%) and the risk of suffering from it in 9 (29%). Regarding the diagnostic criteria for GLIM, 10 (32.3%) had severe malnutrition, 6 (19.4%) had moderate malnutrition, and 15 (48.4%) had a normal nutritional status. The SARC-F questionnaire showed that 16 (51.6%) had a high probability of sarcopenia and according to the FRAIL scale, 27 (87.1%) had suspected frailty.
Conclusions: There is a high prevalence of malnutrition in elderly hospitalized patients. In addition, we observed a high prevalence of other disorders such as sarcopenia and frailty and a significant association between malnutrition and sarcopenia. The high prevalence of malnutrition along with the negative effects it entails makes it necessary to allocate more efforts and resources to its prevention, detection and treatment.
