Introduction: Antimicrobial resistance (AMR) is a significant threat to global public health, although its incidence is higher in less developed regions such as Turkana County in Kenya. The objective of this work was to study the molecular mechanisms involved in the development of AMRs in microorganisms isolated from patients in Turkana. Materials and Methods: A prospective, multicentre, observational study was carried out. Firstly, the isolates were identified through phenotypic and molecular approaches, and their antimicrobial sensitivity was studied in vitro. Next, the genome of selected strains was sequenced and compared with the Comprehensive Antimicrobial Resistance Database (CARD) to obtain the resistome. Results: A total of 13 strains belonging to 8 different species were sequenced, revealing 211 different antimicrobial resistance genes (ARGs). ARGs against quinolones, beta-lactams, and tetracyclines constituted more than 45% of the total and 65.35% were associated with antibiotic efflux. Finally, the correlation between genotype and phenotype varied significantly depending on the type of antibiotic studied. Discussion: The results showed a high presence of multiple resistance genes, which translates into a greater potential for horizontal gene transfer to other pathogens. However, to obtain a more accurate picture and establish measures for the proper and effective use of antibiotics, it is necessary to increase the number of isolates studied.
