Introduction and Objectives The breech presentation or transverse situation occurs in 3-4% of pregnancies and the External Cephalic Version (ECV) is a maneuver whose importance lies in the generation of a more favorable fetal presentation for vaginal delivery, avoiding caesarean section and the complications and economic costs that this technique entails. The main objective of this study is to determine the success rate of ECV at the Hospital General Universitario Dr. Balmis, as well as to study the variables associated with success and the relationship between successful ECV and achieving vaginal delivery. Material and methods A retrospective observational study of a cohort of term pregnant patients with fetuses in non-cephalic presentation who underwent ECV was carried out on 251 patients who underwent the ECV between 2013 and 2021 in the Obstetrics and Gynecology Service of the Hospital General Universitario Dr. Balmis. Statistical analysis of the data was performed with the IBM-SPSS V.23 program. The comparison of the explanatory variables between both groups was carried out using the Chi-square test and Fisher's exact test. To establish the degree of association between the success of the ECV and the variables, the Odds Ratio was used. Finally, a multivariate analysis was performed using binary logistic regression to evaluate the influence of the explanatory variables on the success of ECV. Results The success rate of ECV in our study was 77%. Parity is statistically significant at 95% with ECV success, obtaining an adjusted OR for parity of 1.8 (95% CI 0.9-3.5) with an adjusted p value of 0.056. Likewise, the parity index of amniotic fluid (AFL) shows a significant association, with the ORa being 4.7 (95% CI 1.4-16.4) with an adjusted p value of 0.014 (previous p 0.019). In the case of the number of attempts to perform the technique, we observed a significant association at 95% with the success of the version, with the ORa being 7 (95% CI 1.2-40.1) with an adjusted p value of 0.029 (previous p 0.023), so it is also statistically significant, being associated with the success of ECV. Vaginal delivery was obtained after successful ECV in 54.8% of the cases, so that 137 caesarean sections have been avoided by carrying out 251 versions. Based on this last result, the NNT (number needed to treat) is 1.83, that is, 1.83 ECV must be done to avoid performing 1 cesarean section Conclusions ECV is an effective and safe technique to offer pregnant women at term with fetuses in non-cephalic presentation and its success rate is associated with fewer scheduled caesarean sections at the Hospital General Universitario Dr. Balmis (Alicante). The 3 variables involved in success are parity, amniotic fluid index (AFI), and number of ECV attempts.
