Evaluación del malestar emocional de pacientes y familiares en la Unidad de Cuidados Paliativos de un Hospital General: un estudio piloto

Abstract

El trabajo evalúa el grado de malestar emocional de los pacientes con enfermedad avanzada, ingresados en la Unidad de Cuidados Paliativos de un Hospital General, así como los niveles de ansiedad, depresión y sobrecarga de sus cuidadores principales. El énfasis del estudio está en valorar las posibilidades de evaluación y la idoneidad de los instrumentos seleccionados, dadas las características de esta población en la fase terminal de la vida. Se realizó un muestreo de conveniencia de 25 pacientes entre el 2 de noviembre y el 17 de diciembre de 2015, consiguiendo evaluar a 7 pacientes y 8 cuidadores. Para valorar a los pacientes se utilizó el índice de Barthel, el Cuestionario de Evaluación de Síntomas Edmonton (ESAS) y el Cuestionario de Detección del Malestar Emocional (DME). Para evaluar a los cuidadores se emplearon la Escala Hospitalaria de Ansiedad y Depresión (HADS) y la Escala Zarit reducida. Los resultados indican que existe un alto nivel de malestar emocional en el 71.4% de los pacientes. En los cuidadores evaluados aparecen niveles altos de ansiedad; en cambio, no se observan niveles destacables de sintomatología depresiva. En relación al nivel de sobrecarga del cuidador, el 87.5% de los sujetos puntúa por encima del punto de corte. A pesar de las dificultades que conlleva evaluar a los pacientes con enfermedad avanzada y sus cuidadores, consideramos que se trata de procedimientos que contribuyen a detectar el sufrimiento y promover su bienestar.

This paper evaluates the level of emotional distress in patients with advanced disease in a Palliative Care Unit of a General Hospital, as well as the levels of anxiety, depression and burden of caregivers. The emphasis of the study lies in assessing the possibilities of evaluation and the feasibility of the selected methodological tools, given the characteristics of this population in the end of life. A convenience sampling was used among 25 patients, from 2nd November to 17th December 2015, being able to include in the sample a total of 7 patients and 8 caregivers. In order to evaluate the patients we used the Barthel Index, the Edmonton Symptom Assessment System (ESAS) and the questionnaire of Detection of Emotional Distress (DED). In order to evaluate the caregivers, we used the Hospital Anxiety and Depression Score (HADS) and the reduced Zarit scale. Results indicate that there is a high level of emotional distress among 71.4% of patients. In the caregivers analyzed, high levels of anxiety were found; on the other hand, there were no significant levels of depressive symptomatology. Caregivers were observed to have moderate and high levels of burden, with 87.5% of subjects scoring above the cut point. Despite the difficulties involved in evaluating patients with advanced disease and their caregivers, these procedures contribute to detect suffering and to promote their wellness.
