INTRODUCTION. Despite advances in open and endovascular lower limb revascularization,
major amputation rates remain high in patients with critical limb ischemia.
OBJECTIVES. To analyze the proportion of major amputation patients who had previously
undergone limb revascularization procedures and to determine the number of such attempts,
the types of procedure and the anatomical sectors treated.
MATERIALS AND METHODS. A retrospective review of patients who underwent major
lower limb amputation (supracondylar or infracondylar) at the Hospital General Universitario
Dr. Balmis in Alicante during the year 2022.
RESULTS. During this time, 71 patients underwent surgery for major amputation, 80% of
whom were male, with a mean age of 70 years. Of these, 66% presented three or more
vascular risk factors, with an amputation surgical mortality rate of 17%. Sixty percent of the
patients did not undergo any previous revascularization because most of them had
irreversible ischemia of the limb. Almost two thirds of the revascularizations performed were
endovascular, with the femoro-popliteal sector being the most treated (68%).
CONCLUSIONS. Most patients who end up undergoing major limb amputation do not
undergo prior revascularization procedures due to their deteriorated baseline condition and
the severe extent of their lesions.
