Sarcopenia is a prevalent disease among older adults, especially in patients with heart failure (HF). This contributes to an increased risk of prolonged hospitalization, which is associated with higher mortality rates. This study aims to evaluate the prevalence of sarcopenia and establish a diagnostic algorithm for its detection in HF patients with reduced ejection fraction.
An observational descriptive longitudinal study was conducted at the Heart Failure Unit of the Elda Health Department over a period of 3 months, involving patients with at least one admission for cardiac decompensation in the past 6 months and demonstrated left ventricular systolic dysfunction (LVEF <40%). Data were collected using various validated sarcopenia detection methods recommended by current scientific societies, including the SARC-F questionnaire, grip strength measurement using dynamometry, the chair stand test, gastrocnemius circumference measurement, and bioelectrical impedance analysis.
Upon completion of the study and assessment of the obtained data, a sarcopenia prevalence of 26% was found in the studied sample. Differences in the sensitivity of detection tools were observed, with dynamometry and gastrocnemius circumference measurement being the most useful as they can be applied to all subjects under study. It was concluded that there is a high prevalence of sarcopenia in HF patients with reduced ejection fraction, which is of great relevance as early detection of sarcopenia in these patients is crucial to prevent progression and consequent loss of functionality. Finally, a screening algorithm based on dynamometry and gastrocnemius circumference measurement was proposed, which, combined with the use of bioelectrical impedance analysis in cases where possible due to patient characteristics, will facilitate its detection in clinical practice.
