Introduction
One of the main difficulties in clinical practice is the lack of studies that point out the differences that exist
between the trans binary and non-binary population, in relation to demographic characteristics and
psycho-sanitary needs.
Objectives
This study examines the healthcare trend related to gender affirmation towards the binary and non-binary
transgender population in the Gender Identity Unit, Endocrinology and Nutrition Service of the Dr. Peset
University Hospital between the years 2014 to 2023. In addition, it describes the profile of this population and
identifies possible differences in the medical treatment received, as well as the mental health status between
binary trans and non-binary trans.
Methods
Two different samples were used to carry out the study. On the one hand, and with the aim of analyzing the
health care trend, a sample of 1228 transgender people who were seen in first consultation at the Gender Identity
Unit, Endocrinology and Nutrition Service of the Hospital Universitario Dr. Peset between 2014 was examined
using a linear regression model. On the other hand, a logistic regression model was used with data from a
sample of 120 binary transgender people and 25 non-binary transgender people attended at the same Gender
Identity Unit, between 2022 and 2023, with the aim of analyzing the differences between having received
medical and/or surgical treatment and mental health status.
Results
Using linear regression, it was demonstrated that there is an upward trend in attendance at the Gender Identity
Unit of 20 binary and non-binary transgender individuals per year, representing an 880% increase over the
10-year period from 2014 to 2023. In addition, although no differences were identified either demographically
or related to treatment received, or in clinical diagnosis of depression and/or anxiety, between trans binary and
trans non-binary, we did find that those individuals assigned female at birth showed a significantly higher risk of
depression and/or anxiety (OR=4.94, 95%IC: 1.62-15.05) compared to those assigned male at birth.
Conclusions
The results of this study confirm a significant upward healthcare trend in the binary and non-binary trans
population in the last 10 years in the Gender Identity Unit, Endocrinology and Nutrition Service of the Hospital
Universitario Dr. Peset. Furthermore, in the last two years this population did not differ in the treatment received
(medical and/or surgical) but did differ in a higher prevalence of diagnosis of depression and/or anxiety in the
assigned female at birth group, with respect to the assigned male at birth group. These results should be
interpreted with caution due to the small sample size of the study. We suggest the need for future research of
prospective longitudinal design to better understand the factors influencing care trends and to improve the
quality of care provided to this population.
