Objective The main objective of this study is to identify and review the literature to analyze the homogeneity and describe the methodology in which the determination of fecal fat and the calculation of coefficient of fat absorption (CFA) are performed in various randomized clinical trials of PEI. Methods A bibliographic search was conducted in PubMed and Cochrane Central with a cutoff date of 13-11-2023. The specific terms used for the search in all databases were: (randomize OR randomized) AND "Exocrine Pancreatic Insufficiency" [MESH]. Two independent reviewers completed the search and study selection process, as well as data extraction (MBG and ICT); disagreements were resolved by a third reviewer (EDM). All controlled randomized clinical trials (RCTs) in which fecal fat or coefficient of fat absorption (CFA) were considered as gold standards and the study population was predisposed to PEI (PP-PEI) were selected. Data regarding study design, description of the study population, and methodology for quantifying fecal fat were recorded, and the obtained data were analyzed. Results The determination of fecal fat for the calculation of CFA is a test that is not performed homogeneously nor follows a standardized protocol across studies. The methodology of this test, considered as the gold standard for the diagnosis of PEI, exhibits considerable variability among the different reviewed studies, suggesting in turn a high variability in the results obtained depending on the procedure employed. Conclusions The lack of homogeneity in the procedure of fecal fat determination, along with other limitations of this test, make the CFA not an appropriate gold standard for the diagnosis of PEI, justifying the need for new studies to search for alternatives to the current gold standard.
