Introduction: Giardiasis is a gastrointestinal infectious disease caused by the protozoan Giardia lamblia. It is of significant epidemiological importance due to being a zoonosis with global distribution and prevalence. The disease is transmitted by the ingestion of cysts presents in water and food, by the faecal-oral route by contact with infected animals. Appropriate pharmacological treatment is essential to stop the spread and prevent chronic sequelae such as chronic fatigue and growth retardation.
Objective: A systematic review of available pharmacological treatments is the main objective of this thesis.
Methods: A systematic review of the articles retrieved from the MEDLINE and Embase databases.
Results and Discussion: From the 176 retrieved articles, after applying inclusion and exclusion criteria, 2 clinical trials and 8 systematic reviews were selected that evaluated the efficacy and safety of treatments for giardiasis in humans. The identified treatments include 5-nitroimidazoles (metronidazole, tinidazole, ornidazole, secnidazole), benzimidazole derivatives (albendazole, mebendazole), nitazoxanide, furazolidone, chloroquine, mepacrine and paromomycin. The most effective treatments are the 5-nitroimidazoles, especially metronidazole and tinidazole.
Conclusion: Metronidazole treatment is effective and safe. Tinidazole treatment has the advantage of being administered in a single dose. The rest of the drugs could be the second line in case of therapeutic failure without a consensus on which is more effective. Albendazole is an excellent alternative for use in mass treatments of multiparasitism with a lower incidence of adverse reactions.
