Background: Lack of adherence to treatment is the main cause of poor control in secondary cardiovascular prevention. One of the most important treatments in these patients are statins, as recommended by clinical guidelines. Objective: The aim of this review is to explore the methods used to detect lack of adherence/persistence to statins in patients undergoing secondary cardiovascular prevention. Methods: Following PRISMA recommendations, a systematic review was conducted using the MEDLINE, Embase and Scopus databases. Two researchers independently reviewed titles and abstracts. Once the articles were selected, a review of the full texts was performed. After completing the selection process, the quality of the studies was assessed and data extraction was carried out. Results: A total of 39 articles were selected, of which 71.8% assess adherence, 12.8% persistence, and 15.4% both adherence and persistence. Most of them were cohort studies carried out in the United States, which studied adherence to statins in patients undergoing secondary cardiovascular prevention. The most reported adherence measurement methods were those based the record of drug prescription and supply: proportion of days covered (PDC) and medication possession proportion (MPR). Regarding persistence, the most reported measurement method by the reviewed studies was based on electronic prescription in the hospital setting. The scarcity of studies providing validity indicators for adherence and/or persistence measurement methods was noted. Conclusions: Indirect methods are the most used in the articles in this review, with PDC and MPR being the most frequent. More clinical research is needed to develop a validated and easily applicable method in clinical practice.
