Although there are different questionnaires to evaluate the response to CPAP treatment in patients with OSA, PROMs in DRT have been scarcely evaluated. Objectives: Development of specific PROMs for CPAP and their application in a cohort of OSA patients who start CPAP treatment, to assess as therapeutic response: activity and daytime sleepiness, psychophysical aspects and tolerance to treatment. With the aim of predicting and improving adherence of OSA patients treated with CPAP. Methods: Experts in sleep pathology proposed and screened the initial questions of the TRD scales. Interviews were conducted with OSA patients to assess their understanding. Patients gave informed consent. Their validity and reproducibility were assessed. In a subsequent phase, the questionnaire was applied to a large group of patients through an anonymous application. The results were grouped into three dimensions: Activity and daytime sleepiness Psychophysical aspects and Tolerance to treatment. Data such as age, sex, body mass index (BMI), apnea-hypopnea index (AHI) and time on CPAP therapy were collected. Statistical analysis included descriptive measures, internal consistency test (α-Crombach), correlation and normality. The Student's t-test or Mann Whitney U test was used to compare independent samples, as well as the chi-square test for qualitative samples. Results: After the elaboration of the specific PROMs for CPAP in OSA, the Internal Consistency of the items was evaluated using Cronbach's Alpha. (Total Benefit 0.906 and Tolerance 0.69) and the Reproducibility by CCI (Total Benefit 0.99 and Tolerance 0.949). Subsequently, the final questionnaire was applied to 985 patients aged between 29 and 94 years, mean age 64 ± 11 years, 757 men and 228 women, 63 ± 11 vs. 66 ± 10 (p<0.001), PC 43±3 vs. 38±3 (p<0.001), Epworth 12±4 vs. 11±3 (p=ns). The mean total benefit score obtained was 29±8 points out of 40, with a median of 32 points (8-40) not following a normal distribution, with 75% presenting favorable scores above 24 points. The mean score of the Tolerance to treatment questionnaire was 8.5±3 points out of 30, with a median of 8 points (6-24), not following a normal distribution, with 98.6 % presenting a favorable score lower than 18 points. When comparing the results obtained between men and women by means of chi-square in the dimension of Activity and somnolence, significant differences were found in the reduction of tiredness (p=0.003), greater activity (p= 0.007), less somnolence (p=0.005), controlled pathology (p= 0.0001) and safe driving (p=0.0001), with greater improvement in men. In the Psychophysical dimension, significant differences were found in changes in mood (p=0.025) with greater improvement in men. In the Tolerance to treatment dimension, significant differences were found in claustrophobia (p=0.016), skin wounds (p=0.013), hair restraint (p=0.000), with a higher proportion in women. Conclusions: The PROMS developed are consistent and reproducible. Most patients with OSA report benefits and good tolerance to CPAP. There are gender differences, with men reporting greater improvement and tolerance. They may allow to identify 25% of patients with low perception of benefits, in order to improve their adherence
