Background: Osteoarthritis of the knee is a fairly common chronic degenerative disease, characterised by varying degrees of wear and tear on the articular cartilage. The therapeutic approach can be physical therapy, pharmacological and/or surgical treatment. Within pharmacological therapy, the use of platelet-rich plasma (PRP), administered intra-articularly, has become more and more popular in recent years.
Objective: to determine the clinical effect of PRP in the context of degenerative knee pathology. In addition, to determine the degree of joint degeneration in which PRP treatment is effective, to analyse the progression of the disease with the use of PRP and the existing differences with respect to treatment with hyaluronic acid and corticoids.
Methods: A literature review was conducted using the Pubmed database, selecting a total of 17 studies (13 meta-analyses and 4 reviews), which met the inclusion criteria. The clinical evolution was analysed using the WOMAC, EVA, KOOS and/or IKDC functional scales and the progression of osteoarthritis using the Kellgren-Lawrence (KL) scale.
Results: 3 studies evaluated the functional improvement in grades 1-4 of the Kellgren-Lawrence scale of knee osteoarthritis after treatment with PRP, concluding a clinically significant improvement. Two meta-analyses were reviewed to assess the progression of knee osteoarthritis after intra-articular administration of PRP, showing no significant changes in cartilage thickness. PRP was found to be more effective than hyaluronic acid and corticosteroids, especially in the long term, in 16 and 9 studies, respectively.
Conclusions: PRP treatment reduces pain and produces a functional improvement in all Kellgren-Lawrence grades, although effectiveness decreases from ≥3 KL. Articular cartilage thickness does not change after PRP use. Hyaluronic acid and corticosteroids in the treatment of knee osteoarthritis are less effective than PRP, especially in the long term.
