Background: Cryptogenic stroke (CS) represents a 40% of ischemic strokes. Atrial
fibrillation (AF) is a possible etiology and we may consider screening for AF with longterm
cardiac monitoring tools. AF diagnosis has therapeutic implications and could be
helpful to improve patient’s outcome.
Methods: We have carried out a bibliographic review focusing on the search engine
PubMed, MEDLINE database and Scopus, MEDLINE, EMBASE and COMPEDEX
databases, focusing our review in randomized clinical trials.
Results: This review demonstrates the higher AF detection rate by using ICM in patients
with CS, which progressively increases with de duration of the monitoring, until reach an
AF diagnosis in nearly a third of patients. We also found a higher rate of patients with CS
treated with oral anticoagulation after an episode of subclinical AF diagnosed by ICM.
Its applicability in real-world conditions has been demonstrated. Photopletismography
from smartwatches is another tool for long-term cardiac monitoring and we have seen
from pragmatic studies its feasibility for that purpose.
Discussion: It has been proven that patients with CS may require log-term cardiac
monitoring tools for screening of subclinical AF, which will improve anticoagulant
treatment rate. ICM are the tools with more evidence for that purpose, while more studies
are needed to prove the use of PPG from smartwatches for that aim
