INTRODUCTION: There is some controversy about short-, medium- and long-term rehabilitation protocols after arthroscopic surgical repair. It is proposed that early physiotherapy related to early passive and active movements, as well as pendular and strengthening movements may favor shoulder rehabilitation and functionality, in a safe way, compared to conventional ones. OBJECTIVES: To perform a review of the available literature on what constitute the most current interventional protocols in arthroscopically repaired rotator cuff. MATERIAL AND METHOD: A systematic review of clinical trials available in pubmed, scopus, peddro and cochrane databases was performed from 2012 to the present. Searches filtered results with specific inclusion and exclusion criteria. RESULTS: Eight clinical trials were included. Most of them compare the benefits of applying a specific intervention with early exercise versus a conventional physical therapy plan. Among them, interventions such as isometric activation exercises, mobility exercises, pendular exercises, and digital home therapy were found. The most significant variables were flexion and abduction ROM, Constant and Murley scores, and pain. CONCLUSIONS: Early rehabilitation after arthroscopic cuff repair is associated with some initial improvements in ROM and MMSS functionality, but they eventually equalize by the end of the follow-up period. The development of studies with larger sample sizes and higher methodological quality is needed to reach a more robust conclusion.
