Introduction: Flatfoot is a pathology that affects the height of the medial longitudinal arch, causing a
decrease in its height together with a valgus of the rearfoot and an abduction of the forefoot. On the
other hand, the windlass mechanism described by Hicks is activated when dorsiflexion of the first
metatarsophalangeal joint is performed, producing an elevation of the medial longitudinal arch..
Objectives: The primary objective of this study is to evaluate the effectiveness of an exercise
programme on the activation of the windlass mechanism in a patient with flexible flatfoot. Material and methods: We selected a patient with an adult flexible flatfoot, bilateral and
symptomatic in the right foot, with an absent windlass mechanism. An exercise-based treatment
programme was proposed for 3 weeks, 5 days per week, with a total of 4 exercises. An assessment
was also carried out prior to the start of the programme, just after the end of the programme and at 4
weeks post-treatment, using a total of 5 tests/scales.
Results: Improvements were obtained in the windlass mechanism and in all the scales and assessment
tests carried out, which were maintained 4 weeks after the end of the treatment. Conclusions: A 3-week exercise-based treatment programme is able to produce improvements in the windlass mechanism in a patient with bilateral symptomatic flexible flatfoot.
