Introduction: Chronic ankle instability (CAI) is an heterogeneous condition characterized by perceived instability and episodes of “giving away” of the ankle, that persists for 12 months or more following the initial ankle sprain, and its prevalence reaches to 70%. This pathology is usually due to mechanical and functional impairments secondary to acute ankle sprain, and therapeutic exercise (TE) appears to improve subjective and objective instability. Objectives: Identify and evaluate the effects of TE as a therapeutic procedure in CAI, as well as establish which type of exercise is more appropriate. Material and methods: An electronical research of randomized clinical trials published since 2011 was carried out. These articles were present in the databases PubMed, PEDro, Web of Science, Scopus and ScienceDirect, and they tried to establish the effectiveness of TE in CAI. Results: Eighteen studies were reviewed, of which eleven compared the effect of TE against a control group, and seven made a comparison between different TE programs, trying to specify the most appropriate modality. Static and dynamic postural control, ankle, knee and hip muscle strength, ankle range of motion, functional performance, pain, muscle activation, and perceived instability were generally measured. Conclusion: There is moderate evidence that TE has positive effects, regardless of whether it is applied alone or in combination with other therapies. There is insufficient evidence to determine which modality is better for the management of CAI.
