Introduction: Lumbar instability is known as a loss of stiffness in the spine that leads to an increased mobility in the vertebral segments mainly due to a deconditioning of the stabilizing muscles and the structure of discs, ligaments and joints causing deterioration of function and increased pain. Nowadays, hydrotherapy is used as a treatment in a large number of musculoskeletal conditions with great benefits due to the properties of the aquatic environment. Objective: To demonstrate the efficacy of aquatic therapy or hydrotherapy in the treatment of pain caused by lumbar instability. Material and methods: The concepts "hydrotherapy", "low back pain", "physiotherapy", "water" and "lumbar instability" combined with the Boolean operators "AND" and "OR" were used in the databases Pubmed, Scopus, Cochrane, EMBASE, Web of Science and PEDro. The PRISMA regulations were followed and the Cochrane tool for assessing the risk of bias was used, as well as the PEDro scale to assess the quality of the studies. Results: 12 studies were selected for the review: 11 controlled trials and 1 cross-sectional study. All the studies offered hydrotherapy to at least one experimental group obtaining statistically significant results in reducing pain and improving function. Conclusion: Aquatic therapy is a good treatment proposal for those adult patients who suffer low back pain caused by possible pathologies generated by instability.
