Introduction: Tendinopathies are one of the most common sports / musculoskeletal injuries in the Western world and Achilles tendon injury is one of the most common. Eccentric exercises involve eccentric muscle lengthening of the musculotendinous unit while applying a load and are considered the first choice of conservative treatment to mitigate sequelae and improve quality of life.
Methods: A search was carried out during February and March 2021 in the Pubmed, Medline, Embase, Cochrane and PEDro databases. Taking into account the search criteria, 14 trials were selected. The methodological quality was assessed using the PEDro scale.
Results: The articles have developed results based on the analysis of the possible consequences of the injury, assessing them by means of various scales and tools. Follow-up was somewhat uneven, with 12 weeks being the point with the most revisions in most of the studies.
Conclusions: According to the articles analyzed, eccentric exercise, alone or in combination, is beneficial in reducing the possible consequences of Achilles tendinopathy such as pain, loss of function, strength, endurance, joint range or tissue structural alterations. Eccentric exercise combined with extracorporeal shock wave and vibration training proved to be the best treatment options to obtain positive results. However, there is still a lack of research in this field.
