dc.description.abstractIntroducción: El exceso de peso y la ganancia ponderal inadecuada durante el embarazo representan un grave problema de salud pública por su asociación directa con complicaciones obstétricas severas, como la diabetes gestacional y la macrosomía. Aunque la dieta mediterránea ha demostrado potencial preventivo, persiste cierta controversia científica sobre su efectividad real e independiente del índice de masa corporal pregestacional materno, por lo que resulta imprescindible evaluar esta asociación desde las consultas de Atención Primaria. Objetivos: El objetivo principal de este estudio es describir la adherencia a la dieta mediterránea y evaluar su relación directa con la ganancia de peso gestacional total en mujeres embarazadas. Metodología: Para ello, se propone un estudio observacional analítico de cohorte prospectiva que se desarrollará en el Centro de Salud de Barrio Peral (Cartagena), realizando un seguimiento longitudinal desde el primer trimestre de gestación hasta el puerperio. Se incluirán mujeres gestantes mayores de 18 años con embarazo único y captación precoz antes de la semana catorce, excluyendo a aquellas pacientes que presenten diabetes previa o intolerancias alimentarias severas. Se estima alcanzar un tamaño muestral definitivo de 100 participantes mediante la aplicación de un muestreo consecutivo. La variable principal de exposición será la adherencia dietética evaluada mediante el Cuestionario de Adherencia a la Dieta Mediterránea (MEDAS-14), y la variable dependiente será la ganancia de peso gestacional, categorizada según los criterios del Instituto de Medicina. El análisis estadístico incluirá pruebas bivariantes y modelos de regresión logística binaria multivariante para aislar el efecto protector de la dieta ajustando posibles factores de confusión. Conclusión: Los resultados permitirán optimizar los protocolos preventivos de la matrona, justificando una implementación temprana de educación nutricional o, alternativamente, fundamentando la necesidad estratégica de trasladar la prevención de la obesidad a la etapa preconcepcional.es_ES
dc.description.abstractIntroduction: Excess weight and inadequate gestational weight gain represent a serious public health problem due to their direct association with severe obstetric complications, such as gestational diabetes and macrosomia. Although the Mediterranean diet has shown preventive potential, some scientific controversy persists regarding its real effectiveness independent of maternal pre-pregnancy body mass index, making it essential to evaluate this association from Primary Care consultations. Objectives: The main objective of this study is to describe adherence to the Mediterranean diet and evaluate its direct relationship with total gestational weight gain in pregnant women. Methodology: For this purpose, an observational analytical prospective cohort study is proposed, to be developed at the Barrio Peral Health Centre (Cartagena), conducting a longitudinal follow-up from the first trimester of pregnancy to the puerperium. Pregnant women over 18 years of age with a singleton pregnancy and early recruitment before the fourteenth week will be included, excluding those patients who present previous diabetes or severe food intolerances. An estimated definitive sample size of 100 participants will be reached through the application of consecutive sampling. The main exposure variable will be dietary adherence assessed by the Mediterranean Diet Adherence Screener (MEDAS-14), and the dependent variable will be gestational weight gain, categorised according to the criteria of the Institute of Medicine. Statistical analysis will include bivariate tests and multivariate binary logistic regression models to isolate the protective effect of the diet by adjusting for possible confounding factors. Conclusion: The results will allow optimising midwives' preventive protocols, justifying an early implementation of nutritional education or, alternatively, supporting the strategic need to shift obesity prevention to the preconception stage.es_ES
