Introduction: Abuse of authority involves the improper use of power for personal benefit, creating situations of hostility and injustice towards subordinates. Its manifestations can vary, and it poses a significant problem in hierarchical systems like Medicine. Following the emergence of movements such as #MeToo and #BlackLivesMatter, social awareness has increased, generating a growing research trend. Understanding the situations that occur and identifying the perpetrators is the starting point for addressing these abuses and preventing their negative repercussions not only on the victims but also on the quality of patient care. Objectives: To compile the available evidence on abuse of authority in medical training and to describe the types of situations that occur and the individuals who perpetrate them. Materials and Methods: A scoping review of the scientific literature published in the last decade using various databases (PubMed, Google Scholar, Dialnet). After applying inclusion/exclusion criteria, a total of 74 articles were included in the results. Results: Most studies were quantitative cross-sectional and published in America. Although there are no clear definitions, certain common thematic blocks have been identified: discrimination, physical and verbal abuse, sexual harassment, and general abuse. The prevalence of power abuse is high, and abuses are mostly perpetrated by attending physicians or chiefs (and to a lesser extent by senior residents), although patients also represent a very significant proportion. Conclusions: There are frequent situations of abuse of authority during medical training, and their manifestations are diverse. Further research on this issue is necessary, as well as the introduction of validated tools to describe them and enable comparisons. Recommendations include a wide range of actions, from actions at the personal level or at the leve lof the different services to systemic actions at the cultural level and paradigmatic change in medical training.
