Introduction: Almost a quarter of primary anterior cruciate ligament (ACL) reconstructions ends in failure. These fails will require another technically more demanding surgical approach to reconstruct the damaged graft, which is called “revision surgery” and the patients involved are called recurring. There are several surgical techniques but there is a lack of evidence and treatment options for these cases.
Objectives: To compare the types of ACL revision surgeries for recurring patients. Additionally, to determine the surgical procedure with the most relevant results, and from a physiotherapy perspective, to analyse its implications for post-surgical approach and to propose some treatment basis.
Material and methods: A literature search was conducted, including 11 studies related to ACL revision surgeries.
Results: 9 studies are related to ACL grafts, and 2 are about bone grafts. 2871 subjects are included, providing information about graft types, population, follow-up time, assessment tools and results. 8 studies have a minimum follow-up time of 2 years. 6 studies talk about physiotherapy in less than 135 words.
Conclusions: Autografts offer better results than allograft, specifically BTB and hamstrings. Extra-articular reinforcements provide good results. The IKDC and KOOS scales are the most used outcome measures. The treatment implications will depend on the graft type used and associated damage. There are not rehabilitation models published for these revision surgeries. Melbourne protocol and blood flow restriction therapy are proposed.
