Background: Due to the rapid growth of immigration in the Spanish population since the end of the 20th century and mostly from Latin America, new cases of human lymphotropic virus type 1-2 (HTLV 1-2) have been detected since in regions such as America Central and South remains endemic. Knowing the prevalence of HTLV 1-2 in pregnant women in Central and South America can help to estimate the prevalence of this disease in Spain and thus know the risk of transmission and may allow preventive measures to be taken. Objective: To know the prevalence of HTLV-1 and HTLV-2 in pregnant or pregnant women in Central and South America. To assess the risk of HTLV 1-2 transmission in pregnant women residing in Spain in pregnant or pregnant women in Central and South America. To carry out a qualitative synthesis of studies on the prevalence of HTLV 1 -2 in pregnant women in Central and South America. Methods: To carry out the systematic review and meta-analysis according to the standards established by Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA), searches were carried out in the main databases such as Medline/Pubmed, Embase, Scopus, Scielo, Web of Science. (WOS) and Cochrane Library from March 20, 2022 and last recorded date February 15, 2023. The internal validity and methodological quality of the studies were assessed through the NHLBI (National Heart, Lung, and Blood Institute) which developed the "Quality Assessment Tool for Observational Cohort and Cross-sectional Studies" for studies with a cross-sectional and cohort design, including studies of pregnant women who attended a clinic for HTLV 1-2 screening controls in the Central American region and from the South. Results: The total number of participants among all the studies was 328031. The mean age covered was 24,4 years among the studies that described it. The age ranges of pregnant women were between 12 and 50 years. Most of the studies 25 (73.5%) were from Brazil, 6 studies (17,6%) were from Peru and 3 studies (8,8%) were from Argentina and were screened by ELISA (79,41%) and They were confirmed by Western Blot (59,37%). We identified 377 records in total. A total of 34 articles were finally included in the meta-analysis. The weighted prevalence for the positive screening test for HTLV 1-2 in pregnant women in the South American area calculated with a random effects model was 0,905% (IC 95%: 0,669% - 1,176%) and the weighted prevalence for the confirmatory positive test for HTLV 1-2 was 0.62% (95% CI: 0,46-0,79). The heterogeneity that was detected was high >95% for both HTLV 1-2 confirmation and screening results. Conclusions: In conclusion, the weighted prevalence of HTLV-1/2 obtained was 0.62% in Central and South America. We stress that the prevalence of HTLV 1 infection is higher than the prevalence of HTLV 2 infection in Central and South America and that the latter is highly present in indigenous population groups. The studies with the highest prevalence were presented in non-urban areas and especially in the region of Peru and in the Southeast of Brazil. The risk of HTLV 1-2 infection in pregnant women from Central and South America is between 0.5 and 1%, so HTLV screening should be proposed in immigrant women from Central and South America.
