Introduction: Chagas disease is a parasitic condition caused by a flagellated protozoan called Trypanosoma cruzi. This parasite is transmitted to humans through the bite of triatomine insects, blood transfusions, organ transplants, contaminated food or by vertical transmission, the latter being a problem in Europe and especially in Spain. Objective: The main objective was to carry out a systematic review of the situation of congenital Chagas disease in Spain. Methods: Systematic review of articles obtained from MEDLINE, Embase and Scopus databases. Results and discussion: Of the 173 articles obtained, after applying the inclusion and exclusion criteria, 15 were selected, all of which had been performed in humans and carried out in Spain. After the analysis, it was found that the transmission of congenital Chagas disease is important in Spain, becoming the country with the highest transmission rate in the European Union. Conclusion: Congenital Chagas disease has become a problem for Spain. Early diagnosis and early initiation of treatment can lead to cure rates close to 100% and to a decrease in the number of cases, so it is necessary to regulate screening and control programs for this disease in all the autonomous communities of Spain
