Introduction: WHO and UNICEF recommend breastfeeding during “at least the first 6 months”, due to its reputed benefits. Malnutrition has alongside become a global problem, and fighting against it involves reaching “at least 50% of exclusive breastfeeding at 6 months”. However it currently barely represents 37% worldwide, 39% in Spain and 29% in Catalonia. Peer counseling has proved to be an effective alternative to improve these figures, but it is not an integrated practice in our health system. Objective: To analyze the effectiveness of peer counseling versus regular maternal education regarding the duration and maintenance of exclusive breastfeeding in postpartum women who wish to breastfeed in Lleida, within Primary Care.
Methods: A clinical trial will be carried out by "clusters", through involving postpartum women who wish to breastfeed, with the aim to improve the figures for exclusive breastfeeding maintenance. The healthcare professional responsible for postpartum follow-up will measure the study variable (duration of exclusive breastfeeding). The study group will attend a theoretical-practical lactation session led by counselor mothers, in addition to receiving the usual maternal education provided by the healthcare professional, while the control group will only receive the latter. Differences between these groups — regarding the level of self-efficacy, grasping technique, comfort, level of help required, puerperal mood and breastfeeding skill — will be analyzed. Clinical parameters of the mother and baby — measured both prior to and after the intervention — will also be monitored. Furthermore, subsequent follow-up will allow to evaluate exclusive breastfeeding time in both groups. The analysis of both qualitative and quantitative variables will be univariate and descriptive. Their correlation will be established through the pertinent statistical tests, by setting the significant value at p<0.05 and the confidence interval at 95%.
