Introduction: The clinical interpretation of a Candida spp. isolation in urine can be confusing, which could lead to inappropriate use of antifungals.
Methodology: Descriptive observational study of candiduria episodes (July 2021-2022) at the JM Morales Meseguer Hospital. Patients were classified into asymptomatic and symptomatic based on established criteria, comparing adherence in the management of candiduria with the recommendations of the IDSA guidelines. Results: 135 episodes of candiduria were detected, 28 in community patients and 107 in hospital patients. The mean age was 68.6 years, with 59.3% being male. Risk factors were present in 93.3% of patients, the most frequent being antibiotherapy (77.8%), previous hospitalization (75.5%) and bladder catheterization (59.5%). C. albicans accounted for 60% (80) of the total isolates. Eighty-eight percent of patients had no symptoms of urinary tract infection. Twenty-five percent (34) of the patients received antifungal treatment, with a high percentage (49%) of cases in which candiduria was not managed according to the recommendations given by the IDSA guidelines (P = 0.001). Candiduria treatment was related to those patients who were immunosuppressed (P < 0.001), with pyuria (P < 0.001), candiduria verified by a second urine culture (P < 0.001) and those hospitalized in the last three months (P < 0.001). Conclusions: Candiduria should be included in PROA systems, as it is associated with a high percentage of inadequate treatment.
