Background: Low back pain is a complex and multifactorial disease, it is the leading cause of disability worldwide and the most prevalent musculoskeletal condition. 90% of low back pain is of non-specific origin, so the literature focuses on understanding the mechanisms of maintenance of low back pain; between them, the differences in muscle activation. Objectives: To know, through the scientific literature, the differences in the activation of the lumbar musculature in patients without pain vs patients with low back pain when performing different static and / or dynamic tasks, measured with surface EMG. Material and methods: A bibliographic review was carried out following the PRISMA guidelines, in the databases: MedlinePlus, Pubmed, PEDro, Scopus, ScienceDirect and Web of Science. Cross-sectional studies, published since 2011, that perform an assessment of the activity of the lumbar musculature in subjects with low back pain vs. healthy subjects, using superficial EMG, were reviewed. The modified Newcastle Ottawa Scale (NOS) was used to assess methodological quality. Results/Discussion: A total of 18 cross-sectional studies were included. In patients with low back pain, greater activation of the superficial muscles and less activation of the deep muscles were observed. In addition, they showed a delay in the activation of the deep muscles together with a lower spatial distribution of the activity. The type of study was a limitation. Conclusion: People with low back pain showed a different muscle activation pattern than asymptomatic people.
