Introduction. In the United States, more than 120,000 anterior cruciate ligament (ACL) injuries occur each year. 80% of knee pathologies that require surgery involve this structure. One of the most important sequelae is muscle weakness. After six to twelve months, 64.9% of patients have a lower limb symmetry index greater than 90%. Purpose. The main objective is to review the literature on strengthening protocols after ACL reconstruction. In addition, the aim is to propose the most appropriate one to increase strength and guarantee the safety of the graft. Material and methods. A literature search on the subject was carried out, resulting in a total sample of 472 patients belonging to 10 randomized clinical trials. Results. The trials provide information on the different variables of the rehabilitation program. One of the protocols is proposed as the most appropriate to restore strength without increasing laxity as it is supported by the current scientific literature. Conclusions. An update of the protocols has been performed and all have been found to be safe for the graft. According to the evidence, the protocol of Fukuda TY and collaborators is the most adequate to achieve the objectives of strength and safety. However, more consensus needs to be established regarding exercise programming after ACL reconstruction. The review has a mean of good quality (7.2) on the PEDro scale.
