Background: In spite of patients have the right to know the adverse events (AEs) that take place during their healthcare, caregivers do not always give them this information due to multiples barriers. Objective: To know communication practices of AEs carried out by caregivers with patients in Spain and to determine what variables favour or hinder this kind of communications. Method: An online survey was designed and provided to a total of 1036 caregivers from primary care (n = 593) and hospitals (n = 439) of eight Spanish autonomous regions. Results: A 38.6% of caregivers reported that their institutions fostered systematic communication of AEs to patients. A 41.4% of caregivers had communicated an AE to a patient recently and an 18.5% had been trained to open disclosure. The estimated likelihood of feeling fear of the legal consequences was 7.7 using a 0 to 10 scale. Open disclosure of AEs like a widespread practice at own institution reduced the fear of litigation and increased the likelihood of informing about the event to patient and of apologising to him. Conclusions: It is necessary to encourage a safety culture that fosters open disclosure of AEs to patients and meets caregivers' training needs in this regard.
