Abstract:
La biopsia líquida es una potencial alternativa a la biopsia de tejido con grandes ventajas: es un procedimiento no invasivo, presenta un rápido tiempo de respuesta y permite determinar el perfil molecular tumoral antes y durante el tratamiento, en el seguimiento del paciente y durante la progresión de la enfermedad. Numerosos estudios muestran al ADN circulante tumoral (ctDNA) como un potente predictor de recidiva y supervivencia tras la cirugía y la quimioterapia adyuvante en pacientes con cáncer colorrectal estadio II y III. OBJETIVOS. Determinar el número de copias de alelo mutado en los genes KRAS y BRAF en ctDNA al diagnóstico y postcirugía y estudiar su correlación con la supervivencia global (overall survival, OS), la supervivencia libre de enfermedad (disease-free survival, DFS) y la supervivencia libre de recidiva (recurrence-free survival, RFS) en pacientes con cáncer colorrectal estadio II y III. HIPÓTESIS. El ctDNA es un biomarcador pronóstico válido en pacientes con cáncer colorrectal estadio II y III, que permite estratificar a los pacientes en función del riesgo de recidiva y mortalidad, por lo que podría ser usado para guiar la toma de decisiones terapéuticas. METODOLOGÍA. Estudio analítico observacional prospectivo de los datos obtenidos del análisis del número de copias de ctDNA de las mutaciones en el exón 12 y 13 de KRAS y la mutación V600E en BRAF mediante PCR digital en plasma doble centrifugado de pacientes con cáncer colorrectal estadio II y III al diagnóstico y postcirugía, en el Hospital General Universitario Santa Lucía de Cartagena entre enero y diciembre de 2022.
Liquid biopsy is a potential alternative to tissue biopsy, accounting great advantages: it is a non-invasive process, offers a short response time and allows to determine the tumor molecular profile before and during treatment, during the following-up of the patient and at disease progression. Several studies demonstrate that circulating tumor DNA (ctDNA) is a useful recurrence and survival predictor after surgery and adjuvant chemotherapy in stage II and II colorectal cancer patients. AIM. The aim of this study is to determine the number of copies of KRAS and BRAF genes mutated allele in ctDNA at diagnosis and after surgery and study its correlation to overall survival (OS), disease-free survival (DFS) and recurrence-free survival (RFS) in patients with stage II and III colorectal cancer. HYPOTHESIS. ctDNA is a valid prognostic biomarker in patients with stage II and III colorectal cancer, allowing patients stratification according to the risk of recurrence and mortality, so it could be used to guide therapeutical choice during patient management. METHODOLOGY. Observational prospective study of data obtained from the analysis of exon 12 and 13 KRAS and V600F BRAF mutations using digital PCR in doubled-centrifuged plasma samples from stage II and III colorectal cancer patients at diagnosis and after surgery, at University General Hospital Santa Lucia in Cartagena, from January to December 2022.
