Abstract:
En esta revisión bibliográfica se pretende valorar la evidencia disponible acerca del uso de trastuzumab (agente monoclonal humanizado) como agente único y como adyuvante en combinación con fármacos quimioterapéuticos estándares en el tratamiento del cáncer de mama HER2 (receptor del factor de crecimiento epidérmico humano 2) positivo, ya que, en esta última década, ningún tratamiento sistémico ha alcanzado un beneficio tan elevado en el tratamiento de esta enfermedad como el trastuzumab. Para ello, se ha realizado una búsqueda en las principales bases de datos como PUBMED, The Cochrane Library, EMBASE y SCOPUS, incluyendo únicamente ensayos clínicos que se ajustasen a los criterios de inclusión. Después de analizar detalladamente todos los artículos incluidos en este estudio (3 ensayos clínicos con trastuzumab como agente único + 7 ensayos clínicos en el marco adyuvante con quimioterapia), se ha visto que el trastuzumab se asocia a una mejoría significativa en la supervivencia libre de enfermedad (SLE), la supervivencia global (SG) así como en la calidad de vida de pacientes que sufren esta enfermedad. Además, el riesgo de recurrencia (local o distante) y de la mortalidad fue muy bajo en todos los estudios analizados, a pesar de la heterogeneidad de los mismos. Sin embargo, se ha observado que la toxicidad cardíaca (descenso de la fracción de eyección del ventrículo izquierdo e insuficiencia cardíaca) asociada al uso de trastuzumab se repitió en más de la mitad de los estudios. Este efecto adverso se manifestó de manera muy variada entre las pacientes, observando una mayor incidencia en edades avanzadas, aunque podría revertirse al retirar el trastuzumab. Por último, a pesar de que todavía existen dudas acerca de la pauta de tratamiento, los resultados de estos ensayos apuntan a que un año de trastuzumab es el tiempo estándar de administración.
The main aim of this present review is to assess the available evidence regarding the use of trastuzumab (humanized monoclonal antibody) both as a single agent and as an adjuvant in combination with standard chemotherapeutic drugs in the treatment of HER2 positive breast cancer, since, in the last decade, no systemic treatment has reached such a high benefit in the treatment of this disease as trastuzumab. To do so, a search was made in the main databases such as PUBMED, The Cochrane Library, EMBASE and SCOPUS, including only clinical trials that fit the inclusion criteria. After analyzing in detail all the articles included in this study (3 clinical trials with trastuzumab as single agent + 7 clinical trials in the adjuvant setting with chemotherapy), trastuzumab has been found to be associated with a significant improvement in disease free survival (DFS), overall survival (OS) as well as the quality of life of patients suffering from this disease. Furthermore, the risk of recurrence (local or distant) and mortality was very low in all the analyzed studies, despite their heterogeneity. However, it was objected that cardiac toxicity (decreased in the left ventricular ejection fraction and heart failure) associated with the use of trastuzumab was repeated in more than half of the studies. This adverse effect manifested itself in a very varied way among the patients, observing a higher incidence at advanced ages, although it could be reversed when the trastuzumab was withdrawn. Lastly, although there are still doubts about the treatment regimen, the results of these trials suggest that one year of trastuzumab is the standard time of administration.
